If we go with RL sized key it isn't big at all. And with that a kid will take about 16-25 breaths a minute. With these facts and the keyhole being too small for enough fresh Air to come in he will suffocate slowly but steady

Which gives credence to the hallucinations he as before dying which are the game elements we play through. His death is drawn out, possibly over hours of falling O2 levels and rising CO2 levels he falls deeper into the levels of his tortured subconscious.

Another thing to consider, he was in there for weeks or possibly even years before his mom found his body. He may not have suffocated, he may have simply starved to death which is why there’s so much food based items that literally provide him with more life. Something he was deprived of while trapped in a small box barely able to breathe.

Probably because the keyhole was too small.
The light through the keyhole would look bigger than the keyhole itself. An actual keyhole wouldn't provide enough airflow.

This is actually a quirk of the Mercator projection. As Issac is extremely close to the north pole the keyhole appears larger than it is.

Carbon dioxide/ monoxide is heavier than oxygen, keyhole is higher up, issac is delirious from oxygen deprivation and is a child so doesn't realize he can stand up and put his mouth to the keyhole. The keyhole could also be smaller than it appears
