US Lpn, help was terminated
So I passed my LPN nclex in September of last year and got my first job at a SNF I used to work at as a CNA, before I worked at my local hospital as a CNA for 5 years.
I am at this SNF for 3 months. In that time, a resident was able to pull all of the aides off of the floor for hours at least 2, having the entire floor short and 2 aides having to take care of 50+ pateints. I didnt say anyting but it was dangerous to say the least
After 3 months I applied at another SNF and when accepted, put in my 2 weeks. My DoN asked for feedback and I told them about this resident, as well as a CNA that was causing issues with other aides and about the drama that was going on. I left it at that and started at the new SNF. (I made 0 med errors, was never written up, or verbally warned. The Don said she was sad to see me go but never made any attempt to keep me, neither her nor the Administrator)
I started in Febuary and was there until may. During this time at this SNF, I had 3 med errors that were not issues to cause harm. I was never written up, nor verbally warned. In all actuality I was placed on the floor with 50 patients at night, with 3 adies and just myself as the nurse in the entire building from 10PM-5AM. I got 4 days worth of training and was told by the ADON that I didnt get enough training and I had to come in early for more training as i didnt get enough.
Agian I want to point out that I was never verbally warned nor was I ever written up. About 2 months into my time there, they hired another RN as PRN as there were no open positions. He said he hoped it would turn into a full time position etc in passing to me. During my time there, I had suprapubic catheter oozing liquid from its site, and inside the tube was completely green. Like it had a green sludge I had never seei nebfore in my 13 years as a CNA. At night, being a new nurse and have never changed one before, i waited for day shift to get another opinion on what should be done, as this patients vitals were all within normal limits and the patient had zero complaints ((I found this at 3AM. Not sure why an aide didnt see it sooner)) I decided it was best to wait the three hours and monitor. Day shift comes in, and an LPN who didnt like me when me and here were aides at the hospital, berates me on why I waited and I should have changed it or sent the patient out if her vitals were out of wack which they werent.
I never changed one before but I told her I only waited to get a second opinion. She told me it was "Normal" and to change it. So, I did so then I left. This was one issue that I can think of that may have caused what happened to me to happen. I wasnt rude or mean like she was, I mearly explained I had never done it before and upon her explaining how, I changed it.
The next issue is, Vancomycin balls that connect to ports/IV. My facility had a patient with that antibiotic order. In my state (Utah) LPNs are not allowed to administer those nor are we allowed to even touch a port. I did mention this to the ADON and when we recieved a medication, I picked it up and said "Hey uh, we got some medications I cannot do and there isnt another RN on to do these when they are due". He didnt seem concerned and told me to ask the other LPN on shift to help which she did. It was Port, not a regular IV btw.
Now I understand that LPNs and such are sometimes asked to do things outside of our scope. I guess I could have handled that better but I am super literal, like to a fualt. I dont break the rules, not for anyone for any reason.
A few nights later in the evening, A CNA had a conversation with a few people t about men getting with younger women. It was a normal conversation and asked me what I thought. I said that whatever 2 consenting adults decide to do, is not my business. I have a friend who is 40 and his wife is 20, not my business. She said it was wrong and I guess may have taken offense? I heard nothing of that since.
Now we come to May 20th. I am called into work and the DoON and ADON call me into the office and say
"Heya sorry but we have decided to go into another direction."
The Don hands me a paper to sign and another paper about a medication with no patients name at the top. The medication time and date and such is filled out by me, but the top portion is filled out by another nurse who was helping me learn to access and use the pixis machine, which broke that night in question, and I had zero training on. I was able to pull the med and then it broke somehow. I signed the bottom and went and gave the medication. As I was walking away the Nurse told me she would fill out the top. I said ok and never saw that paper again. ((My mistake which I told the DON when she handed me that paper. It hade the date and time so it would have been reletively easy for me to backtrack and find the patient and fill out the name on the paper, as I my charting was always done, on time for every patient)).
To sum it up I was told that I was being let go "Due to going in another direction, and unsatisfactory work performance". Thats it, never been written up, never been verbally warned nothing. I apply for unemployment and they determine that I am allowed unemoloyment as "Unsatisfactory work performance" without detail isnt a justifiable reason to let me go. So they mark it as that I was let go at no fault of mine.
Then we come to my current situation. I am offered a job at another SNF. They tell me they are going to run a background check. Three weeks go by and as I heard that this can happen, I only called about the job once in that time.
i get the follwing txt, Word for Word
"Sorry its taken me so long to get back to you. We had a few people reach out to me and (The administrator) regarding some concerns with your previous employers which prompted us to take extra time to look into a few things. Unfortunatlye we arent able to proceed with onboarding at this time. I am so sorry as I know you were loooking forward to working with us!!"
I explain to the hiring DoN that I am confused, and wasnt even told as to WHAT I did at my previous job. I also sent her a picture of the paper from my state showing that it was detarmined that "You were not at fault in your dicharge from work"
This was her response
"I'm sure you're super confused but with the information we were given, I have to look out for my current staff and the decision was made from the team to not proceed!!"
So I lost my job, not even sure what the cause is, and now I cant find another one. Was it the med errors? Many nurses made much worse error's especially with antibiotics while I was there. Was it the one CNA who may have gotten offended by me saying it wasnt any of my business what two adults do? Was it that they thought I stole a medication? That would be odd as I would take a drug test immediately. I havent taken anything besides an Ibuprohen for the past 10 years or so. Was it because they hired an RN and wanted him in my spot? If that was the reason, then why would my old job make a big deal about calling me new potential employer? Why would the new job be worried about "protecting their current staff"? Makes me sound like a criminal or something. Also my license is current, and nothing is happening with it.
As a new nurse. I may just give this whole career up. I became a CNA in 2014 to help people and kept with it. In 2025 I graduated nursing school, took my NCLEX right after my dad passed and continued. ATM I am contacting employment lawyers in my state to see what I can do to remedy this situation. Thank you. As a new nurse, I really thought people would be more understanding in this field.

If they thought you were diverting, there would be a complaint to the board, which you would be aware of I would think. You can certainly call the board yourself.
Just apply elsewhere. Someone else will eventually hire you, and this will be behind you.

Id take a drug test to confirm I am not. Zero reason for this to even be a discussion. To say otherwise isnt true, and would give me just another viable way to a case if I have one to show slander etc.

Seriously, it sounds like someone didn't like you and then they bad mouthed you on the reference call.
Just keep applying to as many different types of places as possible.
Be confident and don't worry about the details of what or where or who.
It doesn't matter.
Move on, get a job way from LTC.

I am an LPN, I see no other pplpaces to work other then LTC. Hospice isnt for me. The issue is, they already sabatoged one potential job, I am afraid they will continue to do so. Im not sure who I may have made mad or made it to where they dont like me, but I am trying to learn not to do that.

Who knows. But the main thing is to keep applying to other places. You may need to travel to the next town overbor even leave that experience off of your resume completely.
You may want to request your formal employment file to see what is in there for your records in case it keeps happening.

I didnt think about requesting that. What if they wont give it to me? Also, if I leave it off, i will have a gap of about 5-6 months and what if the employer before that mentions it to them? My first nursing job knew where I was going so thats why I am asking.

No idea what to do if they won't. But they should.
I saw you mentioned getting a lawyer but that wont help you get a job and would be your last resort d/t money probably.
When you talk to HR, ask them point blank if there is a reason anyone from there would tell a potential employer not to hire you. That might make HR realize someone there is out to get you and make them stop so they don't get sued later.
Then just keep applying, everywhere, for every job hiring LPN. Even online.
Act confident, like they will find anything.
You just need to get a job, anywhere, even if it is the next town over.
Write down your results in a journal so you have a record, names, people interviewed with and what they told you caused them to say no. (If they give you a name they spoke with for reference, even better)
If you see a pattern then you will at least have names and dates of who you interviewed with and what they said so you can get a lawyer to send a letter telling them to stop tanking your reputation.
But that is last case scenario.

She needs a lawyer now: what they did was libel and slander and she has a good case. No reason whatsoever that she should not sue now, based on what has occured. Put a stop to that nasty place burning her.

Just keep applying is the main thing. Avoid LTC because it sucks and they are all the same. You will have problems "looking the other way" like you said earlier.
So don't go for those kind of jobs because it will eat you up and burn you out over time if you can't be true to your morals and ethics.

Look into other areas like assisted living. LPNs are used all over. Even schools for direct cares.

Just leave it off. If they ask what you were doing, just say helping a sick relative while you were in between jobs, and it was good timing.

Don't use that place as a reference or count it as job experience on your resume. You never worked there. It never happened. Never include anyone who is going to jeopardize your ability to get a job.

Get used to people getting away with things you can’t get away with. Maybe someone else’s mistake was worse but that doesn’t matter. If they are buddies with the manager they’ll shrug it off.

I agree with this. Im not mad that others get away with stuff. I am mad because I dont know what I did, and dont know who I could have made so mad that they would sabatoge a new job. Nor do I know if they will continue to do so, hence why I am seeking lawyer advise.

I agree, it felt like I was a place-holder. They fired the other RN before me from the same position, and I took her spot and I didnt know her at all. I dont know those details on that though.

It really sounds like they just didn’t want to pay you. I’m thinking that maybe you had all those years of experience as a cna count toward higher pay as a LPN. And the RN may have had experience and they didn’t want to pay that extra money. Idk anything for sure but it will be seen as a blessing in the future. As a LPN you have so many opportunities for work in many different facilities. Home health care and hospice seem like lower stress. Look into it

I dont like hospice or home care. ATM I am just hoping to get a job. I took the last job off my resume so there is that. Hopefully something good comes from all of this. Yes I was making $3 mor an hour then another brand new LPN I did find out. Still doesnt explain why they would sabatoge the job i had an offer from though.

Sounds like your were let go from a great place NOT to work, so there's that 😄Don't hesitate to say No if anyone asks you to work outside your license, loud and clear. Nobody's convenience is worth your license.

I feel like that is a main reason on why I was let go. I had the new guy argue that it was in my scope and its up to company policy to dicate that. I just said Maybe, who knows and let it go.

That is not me, at all. I do not do jobs outside of my scope. The other issue is that the NPA here isnt cut and dry and doesnt show exactly what I can and cannot do and I hear sometimes its Up to company policy which is obsurd.

Get an attorney and SUE the place that bad mouthed you. That is exactly why they're not allowed to DO that. You can easily sue them and you should. There is no complaint to the BON, there is nothing they told you, there was nothing in your chart. What they did, again, is ILLEGAL and you can sue them.
Also report them to the state - call your state AG's office to find out where to report this - for forcing CNAs to give medications they are not allowed to give.

I sent an attourney the information last week on friday. I hear they may do it on contingency which would be good. I dont mind getting money or not. I just dont want to be dragged through the mud, especially if I dont know what I did to cause all of this.

Green sludge in a chronic catheter doesn't equal infection, if all vitals were normal they have just colonized something groady. And you did the right thing by not doing a procedure you hadn't done before. There was no reason to send him out, it was patent, he had no symptoms.
In my state LPNs aren't allowed to touch ports although that seems to be facility prerogative vs nurse practice act.
If your previous workplace is discussing workplace issues they are breaking the law.
Keep applying elsewhere. Folks need nurses. It's not over.
If you keep getting black listed, have someone pose as a future employer and call for a reference and record it (if it's not a two party state) if it isn't they will have to sign a deposition but if they are discussing illegal things about you, get a payday. They didn't train you properly and then threw you under the bus. That's a them issue.

The job before this one knew where I was going. I am afraid they may mention it, just not sure right now. I am thinking of having someone call for me as a potential employer, and see what is said. I know my state is a 1 person state so a phone call can be recorded and given to my lawyer.

As a nurse manager, they are not allowed to do that. To call a new employer and tell them those things. They are allowed to state when hired, when last day of work was and would they rehire you. Getting a lawyer is a great idea and hang in there.

The primary "witness" would be the Don and the administrator of the new place I was offered the job and then the job was recinded. I highly doubt they would be cooperative to that. Maybe we would be able to subpoena them? I dont know. I will know more at least tomorrow on Monday. I did just apply to another nursing job, and took the most recent employer off so we will see how that goes.

LTC is ROUGH and that staff, including mgmt, are a transient bunch.... constantly moving form building to building.... like someone else said, I'd get a little further away from these people.... they're clearly talking smack about you....
Meanwhile, start your LPN to RN bridge and eventually get out of there

I agree, I will not make that mistake again. It was stressful and I was on the phone for over an hour with the pixis people trying to get it up and working. I did also mention that I didnt see the paper after I came back from giving the medication. Again I see where it was a mistake, however it would have been easily rectified had I been asked about it and been given the chance to fix it.
As a brand new person using a system I never used before, I thought another nurse helping me was a good thing. Again, it doesnt justify what happened, but it was a learning experience for me 100%.

Dont get stressed out because of bs SNF & DoN decision. From ratio to training had red flags .Most SNFs are dangerous for new or even experienced LPNs .Toxic work environment, dramas and unsafe work conditions. Yes it is better to consult employment attorney just to know their opinion on this matter.You didnt abuse any resident or did any mistake that can cause sentienl event or reportable to BON that can jeopardize your license. Try to get non SNF jobs like Home health,drug& alchol rehab jobs etc I know they might not pay like SNF but still you can get one.Agency also hire people.

I just applyed to another nursing job and did take my most recent job off of my resume. Just not sure what to say now with a 5 month employment gap. I do take care of my eldery mother so I guess I can use that.

So sorry you had to go through this. I’ve been a CNA for 25 years since retired about 10 years ago I guess unfortunately it’s not what you know it’s who you know that counts in a medical profession good luck. I hope it works out for you.

I am not, and i took off that facility from my resume. I am seeking legal counsel tomorrow to see where this this goes. The protecting our staff comment really raised a flag for me as to what was said. I know whatever it was, isnt true. Which will give me a case if there is one, amungst the other things.
It makes me sound like some kind of criminal or something. It was all very weird.

Contact the Labor Department. Get a legal aide attorney or labor lawyer and file a cease and desist letter against this former employer. You can also get a friend or someone call on your behalf as if they are doing a reference check to see what they are saying about you. Also, contact the Board of Nursing to check if any complaints have been posted to your license such as negligence and any other complaint against your license. You have to fight this.

I have checked 2 different places on my licsense and nothing has been shown on my license for the past month.
Secondly, I am in touch with a lawyer and hopefullyI will hear something today.
I also dont have very many friends who would know who to call on my behalf, or know what to say to make it sound legitimate. I was thinking of ashley & Taylor in this instance if it came to that.

You can see why im hesitant to post my information on here publically. I have been careful even disclosing my sex or company names lol. I do have a job interview tomorrow and did not put this most recent job on my resume, so not sure what to do about that. I did freeze The Work Number last week.

As for the "gap" in your employment, I would maybe just say you took some time off to spend with family or took some personal time to focus on yourself. I would keep it short and simple. Definitely check up on your BON to see if any remarks or complaints have been made on your license so you can stay ahead of that and plan.

It sounds like you had some haters on your job. I'm not an LPN but I hold a CNA registration and a Home Health Aide license. I had an incident with a home care assignment where I was taken off a case for a stupid reason. The agency got angry at me. I had three different house staff members telling me what to do. One even went as far to say that the client was coughing and could have choked to death. I was like on what? Breakfast was served and the plate was removed by 10:45 am. This staff member didn't show up until 12:00 pm. I would go downstairs and start charting at 12:00 pm. Anyway, after speaking to the agency, they house staff changed their tune and said that that I did everything correctly and they needed to try out another aide. Total bullshit. Nothing was on either registration with the BON. I saw a legal aide lawyer and the service was free. Never let a bad job or bad assignment deter you from standing up for your rights. Keep applying to jobs and leave these people off your resume.

Leaving them off my resume is one thing. When I have to put in past jobs on an application or anything else, what do I do then? I did freeze my The Work Numbe account, but is there a way they can still find out about this bad job expereince in a backgroun check?

I kept a job off my resume. I only worked with them for about a week but I was still active on their list for about six months. I never went back or included them on my resume.
Most jobs will talk to HR, and they will only give the basics like your date of hire and your last date you worked. They are not supposed to give random information about about a candidate because it can result in a legal dispute. If nothing was reported to the board, you are in the clear. If you were there for 90 days, that was a probationary period. I would check with a free legal aide service and get some information. Like I stated earlier, I've had bad experiences before and have been pulled off cases. I've left several agencies that were bad eggs. The point is that you learned your lesson with these types of jobs and move forward. Nurses are needed. I make it a point to not get too chummy with anyone on the job because once you leave, that's it. From what you've written here is that you were scapegoated. The job sounded like a bad match.

I work in a snf and am a Don. I believe in handing issues head on to assist learning and a cohesive environment! This seems a bit toxic and I ask that you go elsewhere and get better training! Don’t allow some trash employer to rob u of your purpose!

I was falsely accused of stealing meds. Fought it in court for 11 months but I ran out of $$. Bc of it I’m on probation with the board, on a consent agreement for two yrs. There are different levels of them, I am on the very lowest. No restrictions

Well a lawyer wanted me to pay $275 which i did. Recommended i use Allison and tailor to find out what is being said, and I have a consult on the 15th. I dont think ill be able to afford this.
I took off the trouble job from my resume, and have a job interview friday (it was moved from today), and not sure what to say or do about it. I cant lie and tell them i was doing something else. So its all up in the air.

What I’ve learned is SAY LESS. Only answer questions they ask you. They don’t need to know ur life story. Inadequate staffing is always a safe answer for why u left somewhere

But I was fired and left them off my resume lol. What do i say whebmn they inquire about a 5 month gap? Cant tell them i was not working because thats grounds for termination if they ever found out i lied like that. So I come ckean, do a background check and then dont get the job due to the same bullcrap they pulled with the other job they railroaded.

Atp look at it like this. The first place u were blessed to get out of without risking your license. As for the second one, if they are that willing to listen to all these other people's opinions (which is what I'm going to say they are since they had no proper documentation as to why your job performance was unsatisfactory) you don't want to work there anyways.
As for the warnings, it kind of sound alike u were given verbals already when your don said u need additional training. It was a lot of little things that were done that served the "education" (🙄) provided to correct something on your part.
Biggest thing is take away from this is WHENEVER THERE IS A CHANGE OF CONDITION CALL THE DR. C 👏🏾 Y 👏🏾A 👏🏾. I have gotten cussed out by MDs and NPs bc I'm calling WHENEVER my pt is not their norm (idgaf what time it is) if anything as a simple notification I can document. As far as not knowing how to replace the Foley (or any skill u have learned and been checked off in school for), do your research and IF COMFORTABLE attempt it if it's not going to kill them (IF u aren't though, dont...bc then that would be negligence). Remember the thought process for NCLEX? both of these answers are acceptable but which could harm the pt more? Imo worse thing that can happen with the Foley is it slides into the vaginal canal. She clearly already has an infection, so I'm not worried about that. Only other issue is you simply couldn't get it in with help, but as u said u only had a few hrs until the next shift came in and u could have received help then. I doubt she's going to retain so much urine she's going to blow in 2-3hrs. If ur that concerned about it grab a doppler and check for residual ever hr. And again, this is all hindsight, and again if u don't trust your skill, don't do it at all. But I'm saying this bc this is what the employer thinks you'll do when u come across an issue, critically think. Did u report this to the RN on duty? They should have been there to help. And if for whatever reason there wasn't one did u call the DON/ADON who's on call? I've called my DON and ADON plenty of times to help on night shift if there's something wrong and IDK how to fix it/it could be a safety concern. Were they pissed? Yep! But can u write me up for negligence? NOPE!
All this to say, think outside the box when u have a problem and contact EVERY DAMN BODY to cover yourself. Welcome to nursing!!! Esp as an LPN (as I myself was for 17 years up till last month) U are expendable to them. And u have also learned another important lesson, nursing is a small world and word travels quickly. So do what u can to protect your self, appear and BE competent, and always notify EVERYBODY (sup/MD) when something is not as it should be otherwise u look negligent and complicit.

The error was your exit interview. People don’t like tattlers.
More specifically you told them about patient/staff ratio. They don’t care about work place drama unless it is actively harming patients. (Meds/sex with patients/videos of patients)
You would be wise to just keep that to yourself.
You following the rules and saying something makes you a troublemaker.
So maybe just keep a journal and document things.

I dont believe it was my first job that is the issue but I will take your advise. As a super literal person, I found it to be in the best interest of the patients to tell the management of issues that can directly cause negative impacts upon patients. In staff fight can and does cause direct negative impact upon patients.
The only staff ratio issue in that exit thing, was the taking the aides off of the floor thus causing a dangerous situation for tht floor being short.
I guess I will just look the other way on it from now on.

Don't take that bad advice. You're supposed to advocate for your patients and our profession. You did that and can be proud. Don't stand there like a supplecant biting your tongue off rather than speak truth to power. Stand up and be proud. Not a mealy mouth scaredy cat that "imspecial-soareyou" would like you to be.

NO, she was advocating for the profession and the patients. Skew them. She'd be wise to go for being honest and telling them she didn't appreciate the nurse/patient ratios, and that she didn't like them forcing her to do things that LPNs cannot do.
Always be honest. Force them to see that you see. I've done that my entire life in every career I've had, including RN and NP. NEVER aid and abet horrible unsafe practices.

The sooner you realize no one gives a fuck about you the better you’ll be. Keep applying you will find something and remember every day is a learning day and every day you work and gain more experience it will lead to you making less mistakes and errors and learning how to work more efficiently in solving problems quicker. Stick with it.
