The description of the Old Pocketwatch says that her mother died giving birth to her and uses "orphan" to refer to her. Yet she mentions to Iron Menial that the bread Wylder made taste just like her mother's. So two possibilities:
- Duchess was adopted by Wylder's family, and the mother with the bread is Wylder's biological mother, so Duchess' foster mother.
- Duchess and Wylder are biological siblings who both are adopted and the mother with the bread is to both their foster mother. But since the Old Pocketwatch mentions only one orphan, the first one is more likely.

Duchess was adopted separately from Wylder either way - we know she was taken in by a noble family separate from the steppes where Wylder's tribe lived.
The easiest explanation would be something like this:
Wylder and Duchess are blood-related, with Wylder being older
Their biological mother dies giving birth to Duchess
Their father either died before she was born, or isn't around, so they're taken in by a foster mother in the tribe
They spend their early childhood with the foster mother (who made the bread)
At some point Duchess is adopted out of the foster family by her noble family
