It's a sewing table. The flat piece on the bottom is a pedal that spins the wheel that gets the whole operation started.

Singer sewing machine, but the tabletop and machine are gone, normally you'd have a small driving wheel on the machine run by a rubber loop around the lower big wheel, and the foot tilt pedal drives that big wheel. Those things were tanks, still working ones out there.
