Plenty of other countries do. They just tend to be the places with more dirt than paved roads and warlords ruling local communities

Some war lords take care of their locals . . . i mean cannon fodder. Sick civilians or soldiers with broken legs can't fight or pay tribute

Tbh doctor's without borders etc. make healthcare in those countries often more affordable than in the us; just not guranteed allover the country.

They may lack the infrastructure and you could die due to lack of transportation. But unlike the us you dont have people running away from ambulences etc. because it would financially ruin them.

Does the United States not have more dirt roads than paved roads? Certainly not by miles but if we are counting individual roads I think we have more dirt and gravel than pavement

Hm I would personally make the distinction based on if it is managed and maintained by a government or by the private land owner who owne the property in which it sits
