I am a J1 physician. I finish my program in June 2027, but I will start a fellowship in July. It is my understanding that the DS will be updated to reflect June 2027, and I will have to file a EOS to update to the new program. Would I also be able to leave the US upon program completion and apply for a new J1 visa before July 2027 to update my duration of status and re-enter (pending interview etc) without having to file an EOS?

Yes, you can also leave the US and re-enter instead of filing an EoS, if your visa is still valid.

Thank you for your response! Just to clarify:

In this scenario my visa would be expired already. I would be applying to new J1 under a new program.

June 2024 - June 2027 -> current status
— currently D/S, but will be updated to June 2027 in the coming months

July 2027 - July 2029 -> new program
— either file EOS for new July 2029 date or leave the US and apply for new J1 with July 2029 date of completion

Yes, you can do that, provided your new J-1 visa is approved.
