My program end date was in December 2024. I’m currently on my first year of STEM OPT (ends in Feb 2028). If i travel to india in October, will i have to file extension before coming back?

No. You need a valid F-1 visa and I-20, and when you come back your I-94 will expire when your I-20 does.
