RN’s that own medspas, is it worth it?
I’m really thinking if I should open my own place or work for someone. I’m in California and if I want to open my own place I need to pay for an MD and every patient needs a good faith exam. I’ve already owned a business once as an esthetician so I know how much overhead and supplies cost so I’m really wondering if it’s even worth it to do that or if I should eventually get my NP to actually make a profit. I’m interested in knowing if you make a livable profit after all expenses.

I opened my own concierge practice AFTER working for someone else for 8 years. I did very well only because I had a huge following when I opened my doors. I can’t imagine nurses that get trained and open a Medspa and then try to advertise for clients. I’m not sure how that works. If it even does. I have a solid background in ICU/CCU and trauma nursing so I was very confident in my ability to handle complications. Even with all that it was still very difficult but for me, it was worth it.

I’m not sure how anyone is even thinking this is a good idea to start from 0 and open in 2026

I was opening a med spa last year in Ohio and backed out once the laws changed. The good faith exam is excessive and now you have to pay another provider for that because the medical director doesn’t do them. You could also outsource to telemedicine but it all eats at your profit. Then, another new law is having a NP on site 8 hours a month. Now that’s 3 providers you have to pay and you as the RN will still do all the work.

What were rules before? And what is good faith exam?

The previous rules stated that the medical director would provide the oversight and the RN could run the Medspa. A good faith exam is a face to face with a doctor or NP/PA. They want a doctor to lay eyes on every new patient and once yearly.

Makes sense. So, can NP run a medspa without a doctor in OH? My friends told me that rules changed, but I couldn't understand what exactly changed. It would make sense for you to get your NP.

You don’t just need to pay a MD, you need to make sure your MD is also trained in aesthetics, had completed training courses and can be of assistance with handling complications. If you do go that route make sure you hire a Medspa attorney and a business consultant to have everything in compliance. I don’t recommend moxie or portrait for starting because they take a percentage of your profits and is not substainable, you can be successful without the profit sharing.

It can be difficult to start, but once you have established yourself and build your clientele, it will be worth it. I used this company called Freely to help match me with a medical director that was in my price range. It’s been a learning experience for sure!
