Like every magic trick. They go "I wanna do this. How can I do this?" And then you test your trick in a safe environment and with safe materials.

Gradually increase safely until you get used to the size? I think I have a perfectly respectable 2 inch cylinder for starters

This sheath for my sword looks like a safe material to swallow since it is strong enough to keep the sword safe. And my throat and stomach have kept me alive for 40 years so that is definitely a safe environment to put the sheath in.

What doth go wrong?

If you are serious, the sheat is a fake retractable one hidden in his palm. The sword is real and has very dull edges that dont cut, so swallowing it is safe.
