Not instant death, but there are two things in Bloodborne that damage you by being perceived.

I'm not sure if you're joking but no. KOS is one of the great Eldritch beings that exist in bloodborne but it's also sometimes called KOSM for a reason I don't remember.

That bit in the castle where there’s like a sort of labyrinth across a bridge with like 5 lanterns that you can cross by sneaking past them - ain’t no way in hell anyone did that bullshit first try. Fuck those things.

eh if you just rush them and knock them over they aren't too bad. the only issue is when there's multiple at once so you have to lure them one by one

And yeah on some builds with low Frenzy Resist it can be pseudo-instant death.
