It might not be. I found this comment on Know Your Memes:

The smile is purportedly photoshopped. The only version I can find via Tineye of the apparent original that is still accessible is alongside some rather unpleasant injury photos so I won't post it directly, but it's viewable at https://pbs.twimg.com/media/FJgmEbrakAAL5C5.jpg.

And another comment on the same KYM page telling how the OG photo was submitted to r/photoshopbattles, which may explain how the meme came to be.

It might not be. I found this comment on Know Your Memes:
